Just looking for advice. first: I sent the coupon+ notarized letter by certified mail, no response then received a new statement. Next I sent: coupon+1099A+notorized letter by certified mail. again no response did not get the 1099A back, and was sent yet another statement. now I am sending a conditional acceptance. I am just not sure if I should include copies of the green cards from the first two mailings, does anyone know?

Yes always use copies of all exhibits in the C-A. The original green cards AKA form 3811 will go to the treasury when you file the lein
